1.	Complete the Photography Order Form down to the payment section. PRINT TWO COPIES
2.	Wrap your piece and place in a sturdy box but do not seal the package.
3.	Take the piece and a completed copy of the Photo Order Form to the post office or other mailing service and have it weighed and insured.
4.	Complete the "Payment includes" information at the bottom of the printed form.
5.	Enclose the check and order form with your package and mail to:
